- abstract: This data set combines data from the NM Lead Poisoning Surveillance and Prevention Program and Vital Statistics to provide information on the percentage of children who are receiving a blood lead test by 36 months of age and the number of children with an elevated blood lead level. The data set contains counts by birth cohort year and by NM county of the total number of children born, and the number of children by blood lead level category. The presentation of screening numbers by cohort and NM county identifies children at risk, evaluates screening practices and best testing practices for young children.
- purpose: To provide data for the National Environmental Public Health Tracking Network to allow public health professionals and researchers to evaluate screening practices, monitor progress towards eliminating lead poisoning among children, and identify high risk counties. It is also used to generate measures for the national public portal.
- supplinf: The data are from the New Mexico Lead Poisoning Surveillance and Prevention Program and Vital Statistics for children born in 2009, 2010, 2011, 2012, 2013 or 2014, and receiving a blood lead test before their third birthday. Any level of lead in blood is a notifiable condition in NM and must be reported to the NMDOH per NM Administrative Code 7.4.3.12. In NM, universal blood lead screening is required, rather than targeted screening. The reported tests are either reported as a (1) capillary/filter blood screening or (2) venous blood draw. The laboratory reports are submitted byelectronic HL-7 messages through the New Mexico Health Information Collaborative (NMHIC), electronic download, faxed copies, or postal mail from Clinical Laboratory Improvement Amendments (CLIA) certified facilities, or from clinics utilizing CLIA-waived portable blood lead screening devices. In 2009, 35% of blood tests were reported electronically. The surveillance extract submitted to CDC requires that whole numbers, no decimals be submitted. The rounding protocol for NM is that a result 0.5 or greater is rounded to the next highest whole number and a result less than 0.5 is rounded down to the next lowest whole number, with the exception of results falling in the range 9.5-9.9 which are rounded down to 9 so they will correctly be categorized as less than 10. If a test is reported below the detection limit, the detection limit level is entered as <DL into the surveillance system database and reported as the midpoint of the interval between zero and the detection limit. All results are initially reviewed for levels at or above 10 mcg/dL for children under 16 years of age at collection date. All reports are entered into the SQL Epi Lead Surveillance System database. An elevated blood lead (EBL) case is opened on a lead level of 10 mcg/dL or higher. In addition, lead case management providing medical management and education is provided on cases with an EBL of 5 mcg/dL or higher from a venous sample. Capillary samples of >=10 mcg/dL require a venous confirmatory test. Environmental risk assessments are eligible statewide on a blood lead test of >=15 mcg/dL venous. Risk assessments are completed on addresses where an EBL child spends more than 10 hours per week. On-site assessments are only conducted if exposure sources cannot be determined otherwise. The SQL database has been maintained by NMDOH-EHEB since 2007. Electronic data are available since 1993 (good quality data since 2006). The database contains a field for entry of the Medicaid ID# for linkage to Medicaid eligibility data, however reporting of this data from the providers is sparse. When NM was funded by CLPPP, DOH had a data-sharing agreement with the Human Services Department Medicaid office to provide them with lead screening data linked by name and birth date to the Medicaid eligibility files. By using the child's last name, first name, date of birth, and sex, a linkage can still be made to Medicaid data. It is important to note that some some blood lead test results have missing information on county of residence, which accounts for approximately 1-20% of the data for a given year. These blood lead tests with missing county information are included in the tabulated results shown on the EPHT portal but on the maps.
